The Commodity Error 2025+ was removed to allow for a Single Commodity if Plan was ARH.

Changes that were made:

This ticket updated the Quick WFRP Estimate so that a single ARH Plan commodity was quoted on the Quick WFRP Estimate for 2025+.

This ticket was to update the form with items to be in sync with UW Guidelines, DSSH and base form standards.

Changes that were made:

All changes apply to all years.

1. Revision dates were updated to "11-2024".
2. Page 1 Insured's info label "Applicant/Insured's Name, Mailing and/or Street Address and Other Contact Information" were updated to "Applicant/Insured's Name, Street and/or Mailing Address and Other Contact Information".
3. County was removed from the Page 1 header since it is already listed above the table. It was replaced with AOI.
4. Added Person Type, TIN, Spouse and Signature Authorization to be consistent with the Mint form.
5. The top, right and left borders were removed around the County label and field.

 

6. Date Planted data was updated to print in the correct format, it should be MM/DD/CCYY and it is currently printing DD/MM/CCYY. Also updated the format example under the Date Planted label to "(MM/DD/YYYY)".
7. The Practice label was updated to print under "Practice", "(Seed-To-Seed OR Established)". There was no change to how data prints, only the label was changed.
8. A "Map Field ID" column was added to the table, the column was blank so the user can fill.
9. The Land labels and data were moved to be under each line.
10. Statement labels were left justified.
11. The Privacy Act label was combined to one line from two lines. "Collection of Information and Data (Privacy Act) Statement - Agents, Loss Adjusters and Policyholders" instead of "Collection of Information and Data (Privacy Act) Statement Agents, Loss Adjusters and Policyholders".
12. "Circle either Applicant/Insured or Self-Certifier in the labels" was added above the Signature Section.

4. The Practice label was updated to print under "Practice", "Irrigated (Yes or No)". There was no change to how data prints, only to the label.
5. A "Map Field ID" column was added to the table, the column was blank so the user can fill.
6. Statement labels were left justified.
7. The Privacy Act label was combined to one line from two lines. "Collection of Information and Data (Privacy Act) Statement - Agents, Loss Adjusters and Policyholders" instead of "Collection of Information and Data (Privacy Act) Statement Agents, Loss Adjusters and Policyholders".

9. The below instructions were added under the Signature Section.
"Attach the following:
1. Current Crop Year Aerial Photograph OR Global Positioning System (GPS) Map of Unit: For new insureds, new mint acreage, and unit structure changes; document field acres and locations on the aerial photograph or GPS map.
2. Special Report Showing Adequate Mint Stand Determination: This is required for underwriting report and pre-acceptance purposes and suggested for self-certification purposes. Include all information and calculations used to determine adequate stand. 3. Copy of Actual Production History Report (APH): Include the APH used for the insured crop year or production report for the previous crop year."

This ticket was to update the form to improve user experience for SSN and EIN Reporting Form.

Changes that were made:

All changes apply to all years.

1. Revision date was updated to "11-2024".
2. Page 1 Insured's info label "Applicant / Insured's Name, Mailing and / or Street Address and Other Contact Information" was updated to "Applicant / Insured's Name, Street and or Mailing Address and Other Contact Information".
3. The Certification Statement was updated to use "§" instead of "sect".
4. Form instructions above the table were updated from "persons or entities" to "person(s)". "List all person(s) with a substantial beneficial interest in you as defined in the applicable policy provisions (include landlords or tenants insured under the applicant). If none, state NONE."
5. A sentence was added to the form instructions under the current instruction sentence, "If additional space is needed use reverse side.".
6. Statement labels were left justified.
7. The Privacy Act label was combined to one line from two lines. "Collection of Information and Data (Privacy Act) Statement - Agents, Loss Adjusters and Policyholders" instead of "Collection of Information and Data (Privacy Act) Statement Agents, Loss Adjusters and Policyholders".

This ticket was to do some clean up on the Transfer of Coverage and Right to an Indemnity (TOR) form for MPCI and Livestock.

Changes that were made:

Applicable for all crop years for MPCI and Livestock TOR forms.

1. Revision dates were updated to "11-2024".

2. Statement labels were left justified.

3. The Privacy Act label was combined to one line from two lines. "Collection of Information and Data (Privacy Act) Statement - Agents, Loss Adjusters and Policyholders" instead of "Collection of Information and Data (Privacy Act) Statement Agents, Loss Adjusters and Policyholders".

4. "Certification Statement" label was added above the Certification Statement verbiage.

5. Livestock TOR - The "Mailing and/or Street Address" was updated to "Street and / or Mailing Address".

6. MPCI TOR - The Legal Description label was updated to have the "(For RI/VI..." to start under "Legal Description **".

This ticket was to update the Perennial Combo Form to have the ECO child line printed on the form. The parent line for ECO was being duplicated on the Production Report/Acreage Report and PAW page.

Changes that were made:

Update for all years.

Perennial Combo Form was no longer duplicating parent line when ECO was elected.
